We are now using a program called Rank One. Rank One is the platform that houses the required Physicals and Parental Consent forms for Fayette County schools! Please ensure that all 4 forms in the Physical packet are complete before uploading. Incomplete forms can delay your child being cleared by the Athletic Department in a timely manner before tryouts. Here are the steps to get an account for your student athlete.

1) Complete this survey (You will not be able to find your student in Rank One until you complete steps 1 & 2.)

2) Wait at least 24-48 hours for your student’s ID number to be synced to Rank One.

3) Log in to www.rankone.com to create an account and search for your student. You will then be able to upload their physical and sign the e-forms!
